PEAK HEALTH PLAN RENEWAL, CANCELLATION, AND REFUND POLICY

This Policy governs membership plans, laboratory test orders, renewals, cancellations, and refunds for services provided through ReachYourPeak.Health, operated by Peak Health.

By enrolling in any plan or purchasing services through this website, you acknowledge and agree to the terms set forth below.


LABORATORY TEST ORDER REFUNDS

Laboratory test orders are eligible for a refund within thirty 30 days of purchase, provided that no specimen collection has occurred and no laboratory testing has begun.

Once a blood draw or other biological sample has been collected, or laboratory analysis has commenced, the laboratory test order is deemed fulfilled and is not eligible for refund under any circumstances. This applies regardless of whether test results have been received, reviewed, or accessed.


AUTOMATIC RENEWAL

All month to month membership plans are billed through automatic electronic payment using a valid credit card, debit card, or bank account authorization. Memberships automatically renew each billing cycle on the same date and under the same payment terms established at the time of enrollment.

Prepaid plans, including three 3 month, six 6 month, and twelve 12 month plans, are billed in full at the time of purchase and will automatically renew upon expiration at the same rate and terms unless properly canceled in accordance with this Policy.

Members are solely responsible for monitoring billing dates, renewal dates, and plan expiration.


MEMBERSHIP CANCELLATION

Month to month memberships may be canceled at any time by providing written notice at least thirty 30 days prior to the next scheduled billing date.

All plans require a minimum of thirty 30 days written notice prior to the next billing date to prevent further automatic charges. Cancellation requests received within the thirty 30 day billing period will result in one final payment being processed on the next scheduled billing date.

Following the final payment, members will have thirty 30 days from the last billing date to access and utilize remaining plan services, if applicable.

No refunds will be issued for any payment that has been processed.

Six 6 month and twelve 12 month recurring membership plans require thirty 30 days written notice prior to the next billing date. Early termination of these plans will result in an early cancellation fee equal to one month of the applicable membership rate.


LAPSES AND REACTIVATION

If a membership is canceled and later reactivated, enrollment will be subject to the pricing, plan availability, and terms in effect at the time of reactivation. Prior rates or promotional pricing are not guaranteed.


REFUNDS AND CANCELLATION PROCEDURE

Peak Health does not provide retroactive refunds for membership fees, services rendered, or unused plan benefits.

All cancellation requests must be submitted in writing via email to the official Peak Health contact address designated on the website. Cancellation requests submitted through social media platforms or third party messaging services will not be accepted or processed.


POLICY MODIFICATIONS

Peak Health reserves the right to modify this Policy at any time. Any changes will be effective upon posting to the website. Continued use of services following the posting of changes constitutes acceptance of the revised Policy.
